A merit, a demerit or an incident is recorded against the child, with who saw it and what was done. The points add up on their own, and anything still open is flagged until it is closed.
This is the part of a school system that most often lives in a notebook, which means it is lost the year the teacher leaves. Kept here it is on the child's page next to their attendance and their homework, visible to the staff you decide and to nobody else.
